Abstract

Background: Pelvic magnetic resonance imaging (MRI) is the current state-of-the-art imaging modality for local (re)staging of rectal cancer. Results: However, reporting solely TNM stage according to UICC (Union Internationale Contre le Cancer) does not meet the requirements of our colleagues from the surgical, oncological and radiotherapeutic departments. Radiologists must be able to record relevant findings beyond TNM and convey them in a structured report. The European Society of Gastrointestinal and Abdominal Radiology (ESGAR) published a consensus statement regarding both technical aspects and relevant findings in MRI of rectal cancer. Conclusion This article reviews the technical-practical aspects of rectal MRI (such as patient preparation and MR protocol). Furthermore, typical findings of rectal MRI with their significance for referring colleagues are presented, following the recommendations of ESGAR. In particular, this review focuses on tumor morphology and location, T stage, infiltration of the mesorectal fascia, peritoneum, sphincter complex and pelvic floor as well as signs of extramural vascular infiltration (EMVI) and detection of metastatic lymph nodes.
