Abstract
Background There is a well-established social gradient for the risk of untreated caries. Objectives This study aimed to examine the relationship between untreated caries, three regional socioecomonic determinants and migration background of 6- to 12-year-old primary school children in Vorarlberg, Austria in the school year 2016/2017. Materials and methods Oral health data and age were systematically recorded at the prophylactic group examination conducted by Zahnprophylaxe Vorarlberg GmbH. The binary outcome variable was untreated caries which was defined as having at least one untreated carious tooth (WHO Method). The municipal socioeconomic factors were the parents' educational level, employment status and income. The migration background was measured on school level. Age and urbanization of the municipalities were included as potential confounders. A mixed logistic model was used for the oral health outcome analysis. Results In Vorarlberg in the school year 2016/2017, 35.1% of the primary school children had untreated caries and on average 0.9 untreated decayed teeth. Higher odds for having untreated caries were found for children in municipalities with a higher income, in schools with a higher proportion of children with migration background and in schools located in a rural municipality (reference: urban centers). Conclusions The data situation of oral health and social indicators should be improved on a small-scale-ideally on the individual level. Moreover, existing preventive measures should be evaluated and schools that have not yet participated in preventive measures should be contacted. Measures for vulnerable groups should be planned, implemented and evaluated.
