Abstract

BackgroundThe treatment of pancreatic cancer has made clear progress in recent years. In combination with (neo)adjuvant chemotherapy, surgical resection is the only curative option.ObjectiveThis review summarizes the current standards and possible perspectives in the surgical treatment of pancreatic adenocarcinoma.Material and methodsAselective literature search was performed and a summary of the latest data on the surgical treatment of pancreatic cancer is presented.ResultsSurgical resection of primarily resectable disease is highly standardized in specialized centers. In patients with borderline resectable pancreatic cancer, where an initial R0 resection is not possible, neoadjuvant therapy is applied to achieve secondary resectability. In cases of locally advanced clearly unresectable tumors, sequential treatment strategies including systemic chemotherapy, chemoradiotherapy and surgery are used. Secondary resections can nowadays be performed with low morbidity and favorable outcome in specialized centers.ConclusionThe treatment of borderline resectable and locally advanced pancreatic cancer is an interdisciplinary challenge that should only be performed in specialized pancreatic centers.
