Abstract

In stroke care, glucose and insulin metabolism play a pivotal role in acute management, as well as in long-term risk factor management. The relative risk for cerebrovascular events in patients with pre-existing diabetes was found to be particularly high in women and young adults. When an acute cerebrovascular syndrome is suspected, an emergency CT or MRI of the head is essential to guide further treatment. In case of ischemic stroke, systemic intravenous thrombolysis and mechanical thrombectomy are well-established procedures that also showed efficacy in patients with radiological evidence of tissue-at-risk beyond 6 & x202f;h since symptom onset. Hyperglycemia is a frequent phenomenon after acute stroke that has been shown to be predictive for an unfavorable outcome. However, recent trials do not support intensive glucose management with a therapeutic blood sugar target <130 & x202f;mg/dL.
